What is included in the Accompanying Person’s Registration Fee

• Admission to general areas of the Congress Center (not to scientific sessions)
• Welcome Cocktail and Social Dinner

How to get registered
Those who wish to participate in the Congress are requested to process their on line registration or download the Registration Form (.pdf).

Confirmation
A confirmation message will be then sent by the Organising Secretariat AIM Group International – AIM Congress.
The original invoice will be sent by e-mail.

Payment
Please note that registration will be processed only when accompanied by the total payment.
Registration fee must be paid in advance as follows:

• credit card (Visa, Mastercard, Eurocard, American Express)

• bank transfer to AIM Congress Srl

  Please refer to SHOCK SOCIETY/NAME of the participant and send copy of the bank transfer to the Organising Secretariat by fax or by e-mail.

All participants to the Congress will have the chance to become ESS (European Shock Society) Associate Members. Please see the Society website (www.shocksocieties.org/european_society/officers) for free registration.

Cancellation and Refund Policy
Notification of cancellation must be sent in writing to the Organising Secretariat AIM Group International – Florence Office. Cancellations received:

• by May 15, 2011 - full refund (less a  30% as for administrative charge)
• from May 15 to July 31, 2011 – 50% refund
• after July 31, 2011 – no refund

All approved refunds will be processed within 60 days after the end of the Congress.

Visa Requests
Depending on your citizenship, you may need to obtain a Visa before coming to Italy. Visit the web site www.esteri.it/visti/index_eng.asp – at least 4 months before the Congress dates – in order to find out if it is the case and, consequently, get prepared to collect the necessary documents in due time for the application to the Italian Diplomatic and Consular Representations in your country of residence.
The Congress Organising Secretariat will send the standard Visa invitation letter upon request.
